In consideration of being allowed to participate in any way in related events and activities of the Charles Fink Karate Dojo the undersigned acknowledges, appreciates and agrees that:

1. The risk of injury from the activity involved in this event or activity is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation.
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless the Charles Fink Karate Dojo, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property, whether caused by the negligence of the releasees or otherwise.
5. I understand that the information collected is required by the Charles Fink Karate Dojo for its lawfully authorized activities.
6. I authorize the Charles Fink Karate Dojo, and/or parties designated by the Charles Fink Karate Dojo to take photograph(s), videotape, or digital recording(s) of me and to use any of these in any and all media for its own needs for promotional purposes including, but not limited to, advertising; audiovisual; editorial; exhibition; media relations; posters; publications and web.

Warning: by signing this Agreement, whether physically or electronically, the undersigned ("Participant") and the Ontario Karate Federation ("OKF") irrevocably agree as follows:

1. Status
   As per the Bylaws of the OKF, which the Participant has read and understood, the Participant is part of the OKF but is not a Member of the OKF as Membership is reserved solely for Dojos. The Participant is entitled to all the rights, duties and privileges set forth in the OKF Bylaws for individual Participants.
2. Policies of OKF
   The Participant shall abide by all the Bylaws and Policies of the OKF and are afforded the protections under these Policies and are likewise subject to discipline under the Policies for non-compliance. The Participant has read the Policies of the OKF and has access to these Policies through the OKF website. The Participant shall sign all required forms of the OKF that are applicable. All Participants under the age of eighteen (18) shall be required to have their parent or legal guardian sign these forms.
3. Waiver & Indemnification

Upon acceptance as a Participant of OKF I agree to abide by the policies and procedures of the OKF. As a Participant of OKF I shall uphold the high standards of OKF and shall never do anything to damage the reputation of OKF. I also understand and agree that any violation of this contract may result in discipline linked to the Discipline and Complaints Policy.

I understand that there are risks and dangers inherent in martial arts training and in participating in and/or receiving instruction. I understand and agree that by signing this Waiver/Release, I am assuming full responsibility for any and all risk of personal injury or death or for property damage suffered by me while participating in and/or receiving instruction in Karate. I expressly acknowledge that my participation in Karate may subject me to personal injury or bodily harm and I assume any and all risks of that participation. I also understand that I am receiving instruction from a Dojo that is not owned or controlled by the OKF and that I have no right hold the Ontario Karate Federation and its affiliates, officers, directors, agents employees and anyone connected to the OKF (collectively the "Releasees") liable for any injury or damage which I may suffer while participating in and/or receiving instruction in Karate.

I understand and agree that this Waiver/Release will have the effect of releasing, discharging, waiving and forever relinquishing any and all actions or causes of action that I may have or have had, whether past, present or future, whether known or unknown, and whether anticipated or unanticipated by me, arising out of my participation in and/or receipt of instruction. Knowing this, and in consideration of being permitted to participate in the OKF, I hereby release and agree to indemnify and hold harmless the above-named Releasees individually and their entities, and their officers, agents, principals, partners, shareholders, directors and employees from any and all liability or costs, including attorney fees, associated with or arising from my participation in and/or receipt of instruction. I further understand and agree that this Waiver/Release will be binding on my spouse, my heirs, my personal representative, my assigns, my children, any guardian ad litem and me for said children.

I understand that if I am signing this Waiver/Release on behalf of my minor child, that I will be giving up the same rights for said minor, as I would be giving up if I signed this document on my own behalf. I acknowledge that I have read this Waiver/Release Agreement and that I understand the words and language in it.

4. Privacy Policy
   By providing OKF with your personal information on this membership form and the on-line registration system you are giving consent to OKF to collect and use your personal information for the purposes as set out in the OKF Privacy Policy.

1. Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.The Charles Fink Karate Dojo has put in place preventative measures to reduce the spread of COVID-19; however, the Charles Fink Karate Dojo cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Charles Fink Karate Dojo could increase your risk and your child(ren)’s risk of contracting COVID-19.
2. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you may be exposed to or infected by COVID-19 by attending the Charles Fink Karate Dojo and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Charles Fink Karate Dojo may result from the actions, omissions, or negligence of myself and others, including, but not limited to, the Charles Fink Karate Dojo employees, volunteers, and program participants and their families.
3. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the Charles Fink Karate Dojo. On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless the Charles Fink Karate Dojo employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the school, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Charles Fink Karate Dojo program.
